An eight-year-old boy who suffered a fatal asthma attack missed an appointment at a specialist clinic because of a "mix-up" at a children's hospital, an inquiry heard yesterday.
Owen Charleston, from Cumbernauld, Lanarkshire, died on August 19, 2002. He had been admitted to the Royal Hospital for Sick Children at Yorkhill, Glasgow, in April and August of that year.See the full content of this document
