ACCIDENT investigators have called for safety improvements to be made on single-manned fishing vessels after concluding that the deaths of two men in separate accidents off the coast of Scotland last winter were "entirely avoidable".
A joint report by the Marine Accident Investigation Branch into the deaths of the skippers of the Discovery and Breadwinner found that crew operating without assistance were "at most risk of injury and death" and had escaped previous attempts to drive up safety standards in the industry.See the full content of this document
